We work with a wide range of insurance plans and are actively expanding our accepted coverage, including:

During this transition, patients are still welcome to establish care. Our team will walk you through insurance verification and any available options.

We also offer transparent self-pay options for patients who prefer or need them.

Initial Visit – Establish Care

• Standard Initial Visit – $179
  Preventive care, wellness focus, 1–2 stable chronic conditions
• Comprehensive Initial Visit – $249
  Multiple chronic conditions, complex medical history, extensive counseling or coordination

Visit complexity is determined clinically and discussed transparently.

Follow-Up Visit – Established Patient

• Standard Follow-Up – $119
  Single-issue follow-ups, stable conditions, lab or medication review
• Complex Follow-Up – $169
  Multiple active issues, uncontrolled conditions, extensive counseling

Sick Visit – Acute Care

Sick Visit (Bundled) – $169
Includes provider evaluation plus ONE in-office rapid test if clinically indicated.

Included test options (choose one):
Rapid COVID, Rapid Flu, Rapid Strep, Urinalysis dip, Pregnancy test, Fingerstick glucose

Additional testing is discussed and priced before proceeding.

Not included: Send-out labs, cultures, microscopy, imaging (billed separately or paid directly to the lab).
